When You Arrive
Your Appointment
Please plan to arrive 20 minutes prior to your scheduled appointment time to complete the new patient process. It’s important to be on time for your appointment with your paperwork already completed. A late start in your appointment could result in less time for your treatment and evaluation. During your first visit, the physical therapist will perform a one-hour initial assessment. Subsequent visits will consist of treatments to work towards the goals that you established at the first visit with the physical therapist.
What You Need to Bring
•Completed patient forms.
• Doctor’s referral, if you have one. As of 2019, you can be evaluated and treated by a doctor of physical therapy without a referral.
• Driver’s license
• Comfortable clothes such as shorts/sweats and t-shirt.
• Please refrain from wearing perfumes and scented lotions as we have patients that are very sensitive to these.
Your Evaluation
Your evaluation will take place in a private treatment room, not in a gym environment. Please feel free to ask questions at any point during the evaluation. It is important for us to help you feel comfortable and safe working with our physical therapists. Many of our patients are visiting us for issues that are traditionally difficult to discuss, however we hope you recognize that these are common topics and conditions that we see daily. We respect your privacy and are happy to address any questions and ease nervousness and concerns you may have, especially at your first visit. At N-Balance Physical Therapy, our standard practice is to get to know you so that we may best address the issues you are experiencing holistically and functionally.
