COLLECTION OF PHONE NUMBERS
We collect your phone numbers when you voluntarily provide them to us, for example, when you call in inquiring about physical therapy or wellness services.
We collect your name and phone number when you have filled out our online form on our website for us to contact you for information regarding physical therapy or wellness services.
We collect your name and phone number when you have filled out the “Patient Intake Information” on the patient portal, as a new patient.
USE OF PHONE NUMBERS FOR SMS
Your phone number is primarily used to text you with appointment reminders, cancellations and updates. We will only share your phone number with our SMS provider, subject to their privacy policy. SMS messaging charged may be applied by your carrier.
OPTING OUT OF SMS MESSAGES
If at any time you wish to stop receiving text messages from us, you can opt out by texting “STOP” to opt-out.
Once you have opted out, we will no longer send you any more SMS messages, which means you will no longer receive appointment reminders, cancellation information, or any updates.
CHANGES TO THE POLICY
